Latest revision of passport form

Hiya,

I picked up an APS-1E passport form from my local post office back in February and it is Rev 11/15.

I want to submit my passport application next week and I am wondering if the form I have is still the latest revision or if there is a later one that I will need to use? Has anyone picked up an APS-1E passport form recently?

I did pickup the APS 1 E form but where does it say in the form REV 11/15..
If u point out please..

I picked up a form from the post office a few days ago and it's the same version as yours. I guess that's the latest one we need to use.

No need to worry
Just quick question how did you answer the question in the passport application form...
It's in the first page..
Section B
Question is : Do you also wish to have your birth certificate name noted on your passport?
NB: my name in the passport and birth certificate name is same..
Should I put yes or no
It's kinda confusing

I checked "No" since even I have the same name on my passport and my birth certificate. Saying "Yes" makes sense only if the names are different.

See Note 2, Part B in the guidelines here - https://www.dfa.ie/passports-citizenshi ... uidelines/. The idea is to give people with a different birth name the option to have that name recorded on their passports so that there won't be difficulties in the future due to a name mismatch.

Yea, a bit confusing. I have also answered it no (as Indianzeppelin mentioned) because there isn't another name on my birth cert that I want to include. Perhaps its for people who change their names later I guess.
