Section 9.1 at Permanent residency card application

First time I'd like to apply for permanent UK residence card for myself for my wife and my daughter as an EEA national. We started to live in UK in 2007.
At Section 9.1, Shall I tick Yes and at Section 9.2.
I would give The document ref number and date first time I got in 2007 at Home office Accession State worker Registration. Is this correct?
Section A 9.4 Summary of the relevant EEA nationals activity. Do I need to put all uk employment details or just cover the last 5 years. Do I need to fill up for my wife as well?
Section 9.8 I haven't done College or University course but I have some smaller trainings. Are they important and I need to detai all of them?
