Naming format on passport vs on form

Naming format on passport vs on form

Hi all,
the naturalization form says

"
State your name as it appears on your passport:
Surname/family name:
Other names:
"

However, my name as it appears on my passport is formatted

NAME: Firstname Familyname Middlename

ie, my "middle name" looks like it's part of my surname.

Therefore, I cannot fill in the form "as it appears on my passport"...

because the naming conventions of my original country are different.

How should I fill in this form?

1) Put the correct family name in the 1st box, and put Firstname and other name IN A DIFFERENT ORDER TO MY PASSPORT in the second box?

2) Put my first name in the 2nd box, and all other names in the 1st box?
