Simple Payment question on FLR(FP)

I think I have spent to long on this form today so I must be missing something obvious!
I am referring to the form FLR (FP) form on page 7of 61 Headed Payment Details:FLR(FP)
Basically my girlfriend is the applicant but I will be paying via credit card.

2. Contact name in the UK if different form that of the applicant.
(I'm assuming this is asking for my name as I am paying not my girlfriend)

3. Applicant's full name
(I'm assuming this is for my girlfriend's name.)

4. Date of Birth
(is this the date of birth for me the payer of the fee or my girlfriend's date of birth as she is the applicant).

You may laugh at this but its been a long day!
