Read through the information in the FLR(M) form in the following link...this is what you will be applying under.

The visa fee (postal) is £933 or £1543 if submitted in person. Both options require you to pay £500 NHS Surcharge in addition to the visa fee.
Do you meet the mandatory minimum income level of £18,600 p.a? This can be joint income.
A1 level English is required from a Home Office approved test provider unless you have a degree taught in English.
