Set O application Categories help please.

I had 2x3 years leave – the application was filled via solicitor so I am not sure what form they used and what sort of visa it is called but I believe it (6 years discretionary leave) based on my family living in the uk and British citizen.
My 2nd set of 3 years visa is coming to end this year (dec 2017) , I called the home office to see which application I need to fill and they said I need to use set O application. I don’t know which categories I need to select from the below as it does not have family member of British citizen below – I have listed the Categories from the application, and do I need to fill the section 9 (point base details)

Tier 1 (General) migrant Tier 2 migrant
Tier 1 (Entrepreneur) migrant PBS Dependant
Tier 1 (Entrepreneur) migrant, accelerated Employment not requiring a work permit
Tier 1 (Exceptional Talent) migrant UK ancestry

Other purposes/reasons not covered by other
application forms
and do I need to fill the section 9 (point base details)

Fill only the part relevant to you. You are not a PBS migrant so need to fill any of that
