tier1 enterprenuer child application form question

Hi all,
i am filling tier1 enterprenuer child visa application for my daughter she is applying from india but i am sending all documents from here including filled application form . there is one question that have u ever receive medical treatment from uk ?she born here but she was only 39 days old when i left her in india with in laws now i am confused what should i answer i do yes then then it mention starting and ending date of treatment .and address on that time she didint had any vaccination here only stay one day in hospital after birth and hade one injection in hospital.
second question is with whom you are travelling ? with someone or alone ?
my in laws are applying tourist visa and they have plan to travel with my daughter if they received visa successful. but in colom it say what kind of visa person have who is travelling with you even in laws dont have visa right now !! plz suggest me what should i fill ?

UK born children to PBS migrants are allowed to use NHS for free until 3 months old. So say yes and explain the day at the hospital.
If the in-laws are applying for Tourist visa, you should mention that the baby will be coming back with in laws with the tourist visa

can i pay her IHS CHARGE AND VISA FEES FROM MY HUSBAND CREDIT OR DEBIT CARD IN UK SHE IS APPLYING FROM INDIA ?

IHS is simply a number. No one cares who paid for it from which card

You are asking questions that you know the answer for them to be honest. If there was no travel to the UK then you do not need to put anything there
