I have visited Indian consulate in August 2017 and below are the mandatory documents for birth registration
1)Online Application Form
2)A full version of birth certificate with Parent name.
3)Marriage certificate
4)Declaration form ( You can find it at below location)

5)Parent Passport (Original and copy)
6)Visa /Biometric
7)Address proof
8)Child and both parent's photograph
All photocopies should be self-attested.
