Passport application questions....

I have some questions that i hope someone can help.

I am going to apply for my first ever passport.

QUESTION 1:
My middle name and my surname i use are different from that on my birth certificate.

I have a deed poll which proves my surname change, my driving licence, bank and all else has my new name on it.

But one thing i noticed is, my middle name was never deed poll changed properly. However my driving licence/bank all accepted my NEW middle name.

My old middle name on my birth certificate was (this is an example): JACK
The middle name i've been using for over 10 years is (Again a example): JACKY

To prevent a delay in my application for a passport. Should i put my middle name down as JACK as per my birth cert. OR JACKY as per my driving licence/bank...etc..etc?.

The reason i use JACKY is, i thought that was genuinely my middle name as i didn't have a birth cert at the time. I thought that as its my actual fathers name. I had no idea the birth cert showed as just JACK.

QUESTION 2:
My father was born in Kenya, but to Indian parents. He moved to the UK not long after born. I don't talk to him anymore for over 20 years. Its asking what his nationality is. What would that be?. British-Indian?.

QUESTION 3:
I was born after Jan 1983. Its asking for my parents passport numbers or birth certificates. I do not have either of these as i no longer speak to either of my parents. Could i just put a cover letting explaining this?. Or do i really need to apply for birth cert of my parents as well?.

Advice please?.
