Got IHS refund but I'm not sure that is right

Got IHS refund but I'm not sure that is right

1 month ago I paid everything and made an appointment to get my Tier 4 visa, but at the end I didn't attend the appointment because one of my documents was not in the right format. I requested a refund and started a new application, paid another appointment fee but this time they charged 0.00 for the IHS, I assumed this was because they had registered my previous payment. They also gave me a new IHS reference number that I used in my new application.
Today I received an email saying that my refund was approved including the IHS payment and they mentioned the old reference number.
I am worried that although they didn't mention the new IHS reference, the payment they are going to refund is the only one I have made. Will this hurt my new application? Should I get in touch with UKVI?

Are ECOs obliged to contact me if IHS Is missing?

I submitted a tier 4 visa application but for some reason the IHS was 0.00 pounds. A decision has been taken and they never contacted me, but I have read in the official website and many other sites that when this happens an officer should ask for the payment by email.
Can they simply refuse or reject my application without contacting me (would this be allowed by the immigration rules)? Would this be a case for administrative review?
