I am in the process of applying for a DCPR and I am putting together all of the required evidence for my activities as a qualified person.
I wanted to check that I was on the right track. I have been mostly a student over the 5 years (BA and MSc degrees, 3 + 2), with a summer in the middle as a self-sufficient person, and with the last 4 months of the qualifying 5 years spent also as a worker (since I had a full-time job).
I have written up a cover letter explaining briefly how my situation works out (basically, a longer version of what I wrote above). I have also included two tables in the cover letter to explain how each period within the 5 years is evidenced, with one table for proof of residence and one table for proof of activity.
What they look like:

For my student periods, I am including:
* university letters and diploma
* Italian EHIC expiring in two years and letter from Italy confirming I've had an EHIC since 2004
* bank statements (current account) (every 8-12 months)
* bank statements (savings account, showing £2,000+ in each statement) (every 8-12 months)
* letter from my parents confirming they were supporting me financially
* letter from myself declaring I had sufficient resources to support myself not to become a burden on the UK's social assistance system
* P60s for my side job
* student loans letters
* student bursary letters
For my self-sufficient periods (4 months total), I am including:
* Italian EHIC, and letter confirming I've had it since 2004
* bank statements (current account) for each month of self-sufficient activity
* bank statements (savings account, showing £3,000+ in each statement) for each month
* letter from my parents confirming they were supporting me financially (I don't think they will consider this but I am including them anyway)
* letter from myself declaring I had sufficient resources to support myself not to become a burden on the UK's social assistance system (I don't think they will consider this but I am including them anyway)
For my worker period, I am including:
* payslips for each month spent in this activity (no P60 as I only started this job in June 2017, and I only have payslips to show this)
So my questions would be:
Am I correct in splitting the time in my cover letter in such a way? 07/09/12 to 10/09/12, 11/09/12 to 01/06/15, 02/06/15 to blah blah blah. I am assuming these date splits would come together as a continuous period of time.
Should I take out the letter confirming self-sufficiency and letter from my parents in the self-sufficient period? The guidance notes do not list these outright as ideal evidence for this type of qualified activity, but is there any harm in keeping them alongside other pieces of evidence?
For the worker activity, should I also add monthly bank statements showing receipt of my salary or are the payslips enough?
In the EEA(PR) form, in Section 5 (Residence in the UK), they ask for the first time I entered the UK. I took it to assume the initial date of entry for residence purposes, therefore I have not used the first date I entered the UK entirely, as it was for tourism and not for residence. However, just to be safe I mentioned this in the cover letter and mentioned the exact dates I was in the UK there previous to this 5 year period of qualified residence. Should I do something else?
In the form, they also ask for a complete list of travel outside the UK. I have spent some time looking through my records and I believe I have everything in a nice table I will add as an appendix to the form. However, I am not 100% sure I remembered everything, as it was a retrospective exercise and I may well have lost some travel records. I have made a note of this in my cover letter, stating that any omissions are inadvertent, and are due to the fact I wasn't actively keeping a record of every trip (I wasn't required to). Is that sufficient?
Student council tax exemption/discount - not a state benefit for the purposes of applying for permanent residence, right?
