stolen / lost my EU Permanent resident card

Hi guys, I have lost my EU Permanent resident card, i might lost or some stolen it with my wallet, the question are:

1. How can i apply for replacement EU PR (as i am EU family member)
2. What documents i need to submit with application.
3. Time line

i can u plz help me i lost my Permanent Residence card too, i recently got PR two months ago and lost it, plz tell me which documents u sent with ur application to get replacement, do i need to send again all my pay slips bank letters, wife passport etc Thanks
