If im applying for indefinite leave to remain in the UK as a person who was refused asylum after 6 years -
1. What form do i complete?
2. What supporting documents can I submit?
3. Can I use the premium service?
4. Do I need english test or life in the uk.
5. Do I pay the health surcharge fee?
I was granted my first 3yrs in November 2011

CMOSUK, thank you very much. Your input is greatly appreciated. 1 more question if you don't mind. I have looked at the DL form and there is a question for ihs reference number... Do I leave it blank? I went through the direct.gov website to check but the drop down lists don't have my visa route.
I would leave it blank, the application it self can be used for further leave to remain (extend current visa) in which case you will need to pay the NHS surcharge and will get a Ihs number.
But if it is for settlement, you do not need to pay for the NHS surcharge.
As you are applying for settlement(ILR) after 6 years of DLR you do not need it. DLR is granted outside the immigration rules and therefore it might not be covered in the general visas and applications under the immigration rules.

You’ll be able to use the NHS without paying the surcharge or getting a reference number if:
