Question reg FLR M extension

Hi Friends,

I am applying for FLR(M) extension for my spouse after the initial 33 months leave(expires soon).I have some doubts. Pls can any one help.

Q1.Spouse or children of a settled person has to pay NHS surcharge during the extension?They are in UK now with initial 33 months leave.and is from India.
Reason I am asking is, while I was checking 'how much you should pay calculator', neither of the 3 option shows settled person/citizen option under

'About the person you are joining in the UK' section.It shows all visa types only..None of them are of ILR or citizen

Q2.Are they able to apply the extension application 2 months before the current leave expires?
