Hi,
I'm hoping someone can help me. I've just completed 6 years discretionary leave and is now applying for my indefinite leave to remain. I was granted this based on the fact that my daughter was born here. We both didn't have a visa but got discretionary base on the fact this is the only life she knew and I was her some carer( she has since been neutralized as British and I'm still her only parent/ guardian) Can you please advise me on what documents to provide and if Set (O) is the correct form for me to use.

Yes, Set (O) is the correct application form for applying ILR based on 6 years of DLR.
Normally the circumstances prevailing at the time and reason for grant should still be the same and there should have not been any significant changes.
In saying the documents required some of them are outlined in the application form, such as passport, BRP cards and 2 passport photographs.
You would also need maybe some letters or even covering letters in regards to you and your daughter and on how you are a single parent and I suppose how you are the only one looking after her.
