Applicant with Valid Visitor Visa

Hi,
I am an applicant who already has a valid UK Visitor Visa, and have travelled recently for 2 weeks for business purpose, along with family. I have now begun the process to apply for Tier 2 Visa (with family).

1. The CoS was issued with start date of mid Oct which has already passed. Is there a way to edit the start date Or the sponsor note the only method? My sponsor put a note for the start date extension - does this result in start date being updated at some point by UKBA or will is remain the same as when originally CoS was created, with a sponsor note?

2. In the application there is a question if applicant previously had any valid UK Visa. It is not clear if this question is about immigrant or all visas - thus, I have answered this as yes. Hope this is okay?

3. System asks if I have stayed an UK address in the past - since I travelled with family and stayed at an apartment for 2 weeks, I have answered this as Yes and given apartment address. Is this correct?

4. On the points claim area, I believe only one of points related to CoS should be answers (CoS Shortage Occ./RLMT/Salary). Since the job code does fall into shortage, but the CoS was issued with RLTM, I think I need to claim 30 points against RLTM question - just wanted to reconfirm.

5. For the English language requirement I am applying via UK Equivalent degree. In this section system asks for start and end dates - does this need to be exact?
