IAFT-6 - Appeal against elederly dependent's entry clearance

Hi,

I am looking to make an appeal against the decision of ECO and have been advised by the UKVI to submit the appeal using IAFT-6 Form *2014V5 - Copy provided by the UKVI

For the online, I am unable to locate this option as it asks questions and based on my responses takes me to IAFT-2

It was a settlement visa application and I did not made any human rights claim or is it that adult dependent applications fall under this category automatically?

If so, is it right to choose teh following opton:

- I have received an appealable decision on any other application and the decision is dated on or after 6 April 2015
- Are you appealing from within the United Kingdom? No
- Is your appeal against a decision of an Entry Clearance Officer? Yes
- against a refusal your human rights claim.- Yes

I am getting very confused and IAFT-6 version that was posted to me and the one available online are very different .... .can someone please help?
