Dear Gurus,
Please clarify what is " Start Date with your business (if this differs from start date in job)." in the second column of job table in the application form. I have two employees in each job.
An example please.

If you have an employee who started as a Admin assistant and later became a Sales assistant, those dates would be different.

Start Date with your business (if this differs from start date in job): 04/01/2015
Start Date in job: 04/01/2015
Same dates !

"...if differs from ..." just means you don't need to enter anything in that column if it is the same as the Start Date

Employee 1 - started on 21/09/2015 and finished on 30/09/2016
Employee 2 - started on 1/11/2016 and finished on 30/10/2017

from your comments I understood:
filling the table as follows:

Date Post Created:: 21/09/2015

Employee 1:
Start Date with your business (if this differs from start date in job): Empty
Start Date: 21/09/2015

Employee 2:
Start Date with your business (if this differs from start date in job): 1/11/2016
Start Date: 1/11/2016

Is that right ?

You would leave "Start Date with your business" blank for Employee 2 also. But again, I don't see this as critical
