What Form Should one Fill In?

Dear All
With so many different form I am not sure what form should one use to apply for residence card for a dependent Child of a EU Citizen with PR both currently living in UK?

What is the child's nationality?

Also, how old is the child?
Does the EU citizen hold or has he acquired British citizenship?

Father Non EU acquired British Citizenship Through length in UK.
Mother EU Citizen With PR.
Child EU Citizen born abroad, in UK for 7 Years and since 4 years old.
