Spouse visa extension online application 2017

Hi I have filled in the spouse visa extension form online from inside the UK. My question is do we need to print and send this form with our documents? Or do we just need to send the required documents without this application form ? Also we have done out finger prints in the post office . Do we need to send any detail of that ? As it's not mentioned in the checklist ? Thank you
