ILR for PBS Dependent - Section 5 Queries

Hello Good people,

I am hoping if somebody could provide me some guidance on form Set(O). I was granted ILR in 2013 via Tier2 ICT route and I am now applying ILR for my wife. My wife entered in the UK as my dependent in Nov 2012 as PBS dependent and her leave has been extended twice since then.
Both, my wife and I have been working as IT contractors, operating via our own separate limited companies. Although my wife is on career break for last 6 months. Her company is still active as she intends to start work soon. She has been drawing salary regularly and a minimal dividend.
I am looking at Form Set (O) and have some questions on section 5. I presume this section must be filled from my wife's perspective and therefore only her details are needed to be filled.

1. We own a house and my wife's name is on the mortgage but I make the payments so therefore the answer to question 5.2 will be zero?
The question is Do you or your partner, or both, pay any rent or mortgage for your home? If so, how much do you pay each month?

2. Question 5.3, what is your pay each month after tax and deductions - should this be just the salary drawn as a director or the dividends too? She draws nominal salary and I am one taking caring of all the expenses.

3. Everything else in section 5, after question 5.5 isn't applicable to her as she is applying as a PBS dependent?

In terms of documents, I intend to provide the following.

1. last 2 years cohabitation proofs (will keep the remaining 3 as reserve, if requested)
2. Last 6 months bank statements (Savings, Current etc) for me only (or should it be my wife's statements too)?
3. A letter from my accountant, detailing salary and dividends drawn in last 12 months.
4. LIUK and NARIC letter and the Degree looked at by NARIC
5. Marriage Certificate
6. Photographs etc

I also intend to keep the following with me, just in case the caseworker asks for it.

1. P60 and Tax return proofs, last 5 years for both (my wife and I).
2. Limited company return proofs for both.
3. Company house returns and director details
4. Personal Bank statements for additional 6 months (for both).
5. Business Bank account statements (Limited Company, for both)

Could somebody please have a look and advise on the above? I am going to Solihull PSC for the application.

Please could somebody advise? Questions 1 and 2 are of the prime concern at the moment.
