EEA family permit, scanning documents

Dear All,

I hope any one could help me with the new scanning documents process for the EEA family.

I am Egyptian living in uk depend on a eea family permit as my wife is Hungarian.

I applied for my mum in Egypt to share us through eea family permit but I'm ignorant with the new system.

We received a refusal mentioning that we didn't provide my marriage certificate or a declaration that we will accompany her even we gave them these documents and it should be sent to immigration office.
My questions
" is there a check list for documents scanned like before?"
" How can i complain against the employee who scanned the documents and didn't give us any check list?"
Any way i will apply again but need these information for not to be delayed any more.
