Tier 2 statement of leaves

I'm eligible for apply for ilr this april and part of this application requires me to submit copies of leaves i've taken during 5 year period.

My first firm i worked for got sold to another company and again it got sold to a second firm within the past 3 years so in effect the management and hr side has changed a lot.

I don't know how to get a statement of my leaves during my period with them. How do i proceed with this one?
