Tier 2 General Main Applicant ILR Set (0) queries

Hi,

I have a premium service appointment scheduled for 25th Jan next week at Home Office Croydon. I have received couple of letters from my employers, and 1 more pending/to be received later this week. I am now trying to complete the Set(0) form, and am stuck on the following:

1. Page 18/Question 5.6 "Rate of pay for this SOC code stated in the codes of practice (hourly rate or annual salary)"
I am aware of my SOC code, but neither my employer or UKVI is able to help me on this question. Does it refer to the salary rates mentioned on the Codes of Practice for Skilled Workers document against my SOC code?

2. I was on Tier 2 ICT prior to Tier 2 General. There is a point on Set (0) form page 77 that says "For the period before you were granted leave as a Tier 2 migrant, documents showing that you met the relevant requirements of the immigration rules".
What documents do i need to show here? I have the original stamped Tier 2 ICT visa with immigration stamp when I entered UK for the first time - will that be sufficient?

3. To confirm, a Parking Charge Notification (PCN) is not considered a penalty?

4. Since I have already made the payment to UKVI while booking the premium appointment,I don't need to fill up the Payment page on Set(0)form. Correct?
