IMM 5669 Schedule A Expired

Hi All,

one of my friends has been asked to provide the IMM5669 form for her spouse once again as CIC has stated that it is expired.

They are worried as the whole family had submitted the application at the same day. They are struggling to understand that why the only form has expired. also CIC has given no information on any missing information if there was any

Any help?
