A few questions regarding SET(O) Ancestry

Hi - im currently completing set(O) form for UK ancestry ILR as an Australian who has been in the Uk for 10 years as of 5.4.18. Originally came in on an ancestral visa then extende ancestral visa for another 5 years and have already had my biometrics done.
I have some questions please:
1. being self-employed and working as a contractor (on the CIS scheme) is it suffice to submit last 3 months of bank statements as proof of income and a 2016/17 self assessment tax form?
2. i dont see anywhere on the form where i am asked to provide my own birth certificate or a birth certificate to prove my grandparent was born in the UK. has this requirement been lifted now? because they were previuosly submitted to gain original ancestry visa?
