General information EEA permanent permot

I am new to this forum.
I need some information about EEA permanent card.i am Portuguese and lived in uk from date 08/09/2013 and I am continue working From October 2013.i have also my registration certificate issued in year 2013.
My wife and son also join with me uk from date 08/09/2013.When my family enter they have valid EEA family permit and both are Indian citizen.
After my family have valid residents permit .
My son is 18 years old now.
In year 2016 my got Portuguese passport .
And lost Indian citizenship.
So I need below guidelines.
1.we enters in uk on date 08/09/2013 so which date we complete 5 years in uk and on which date apply for permanent residence card.
2.can my son also apply for permanent residence residence card as a my family permit.
3.which documents need for it.
