I applied for citizenship for my son (MN1), my wife & 1 (AN1) online - well they dont name the forms MN, AN etc online anyways, on 21st March. Then sent docs via NDRS on 23 Mar.
Have just received an email from FMT saying:

I refer to the application for registration as a British citizen of <>.
Before we can proceed further, we require the consent of <> parents/guardians.
Please complete the attached slip and return it to this office by return of post to Department 2, The Capital Building, Liverpool L3 9PP. Please respond within 14 days of this message. An application is invalid unless accompanied by parental consent, failure to reply by the due date will result in the application being rejected.

This is no issues - but its strange that they did not print this consent with my online application - and neither did they put it on the checklist, nor did the NDRS registrar spotted / asked for this.
