which Visa application form fill for mother to visit uk from Pakistan

Hello guys,

I am planning to invite my mother from Pakistan to visit us (me eu-family member have 5 years uk residence card and my EU-SPOUSE) in uk. therefore i have following questions:

1. which application form i have to fill for her? I have found 2 forms one is Visa4UK and other one is simple visitor visa form.
2. she have 6 month bank statement, house on her name and rental income for commercial property. at the moment living with my brother (student) is any other documents she need ?
3. documents from my side will be (bank statement, council tax bill, proof of employment, 3 months payslips and cover letter from my spouse that mother will live with us). do i have to send all original documents by courier or scan copies will be enough ?

please guide me further is i am missing any important documents. thank you
