RESIDENCE CARD AND REGESTRATION CERTIFICATE

HELLO THERE
i am a non EEA national, i am applying for residence card for me and registration certificate for my wife in the same time
i am doing the application myself without any legal advice as they required too much money the questions here are:

1- for the registration certificate and the residence card are they going to be in the same form or i need to have two applications because the application for the same purpose
2-she has a child and in C-CHILDREN 5.13 I ANSWERED YES because we have a child who i am not the parent
and in 5.14 is the child included in this application?
i answered no
and they ask me to explain why
