Set(O) doubts: Tier-2 General to ILR (5-year route)

Hello to all wonderful members here. This forum has always been very helpful and I hope I will get answers to my doubts soon again.
I will be applying with family for ILR around June. My Tier-2 General was issued first on 27th June 2013 (we entered on 14th July 2013), but I believe I can apply 28-days before 27th June 2018 (as per 28-day rule). Please correct if I am wrong.
I have following doubts in Form Set(O):

5.7: Income source: Need to mention employer name or just say "Employer"?
7.14: Family ties with home country: Should I mention things like "Parents, In-Laws, Cousins live there"? or something else. It is in table format.
10.13: Date when last fingerprints were taken for a UKBA application: How can I find that? I did that when I applied for visa extension.
12D, page 78: For the period before you were granted leave as a Tier 2 migrant, documents showing that you met the relevant requirements of the immigration rule: I don't understand this at all. Can someone please explain? And what is needed here?

Up to your decision, my strategy was I applied myself (main applicant first) then my dependent. This is because touch wood if I got rejected, also would be my dependent, and thus a very big dump sum would be gone (to me). So if you will go to premium service and you can get the decision on the same day (mostly). To me this is just a risk management, if you are very confident of course up to your choice.

You are right is just something like that.

Yes. Then this is the date of your biometric done when you do the visa extension.

These are the documents you used for your Tier 2 entry clearance application. I don't have all of them as well but just try to get as many as you can. I don't think these documents are important for your ILR decision anyway.

Have you got it recently?
I have a small list of docs..Can you please check if I have missed anything?

1. Employer letter: Statement you were granted leaves by your employer and they were paid leaves
2. Employer letter: they need to mention you are employed with them since xxxxxx and your designation is xxxxxx and corresponding UKBA equivalent and salary requirements , your salary
And then add a line stating that you are required for foreseeable future in the company
3. Employer letter about payslips being online only: Letter on company headed paper and signed by a senior official, confirming the payslip is authentic.
4. Payslips (March to May)
5. bank statements (1-3 months, original), dated no earlier than one calendar month before the date of the application
6. Child's birth certificate
7. Marriage Certificate
8. Council-Tax, Bank statements with both names (old+Recent), Home papers - To show we've been living together
9. 2 photos each
10. One filled Form
11. Fee payment receipt
12. Appointment print
13. Life in the UK: Original 2
14. English Test/UKNARIC - For both myself and wife
15. Passport - Old and New
16. BRP - 3
18. All Degree/Marksheets
19. Entry clearance documents: TB test, Point calculation sheet etc.
20. Any other document related to UK..Will take P60s, Wife's past employment record, All the old bank statements etc.

Hi,

Just updating as I got my ILR approved yesterday. Might be helpful for someone.

The documents I provided are mentioned below. I did it at Premium Service Centre, Sheffield at 9am. The staff was very polite and quick. They have very good space for kids/families to stay and wait.
I provided only originals (although I had photocopies) but they didn't ask. No questions asked, the approval was given within 1.5 hours for all 3 of us :).

1A. Cover letter: Table of absences and mentioning that my qualifying period includes the time between Entry clearance granted and my entry into the UK (between 27th June 2013 and 14th July 2013).
1. Employer letter: Statement you were granted leaves by your employer and they were paid leaves
2. Employer letter: they need to mention you are employed with them since xxxxxx and your designation is xxxxxx and corresponding UKBA equivalent and salary requirements , your salary
And then add a line stating that you are required for foreseeable future in the company
3. Employer letter about payslips being online only: Letter on company headed paper and signed by a senior official, confirming the payslip is authentic.
4. Payslips (only 1 month)
5. bank statements (1-3 months, original), dated no earlier than one calendar month before the date of the application
6. Child's birth certificate
7. Marriage Certificate
8. Council-Tax, Bank statements with both names (old+Recent), Home papers - To show we've been living together
9. 2 photos each
10. One filled Form
11. Fee payment receipt and Checklist
12. Appointment print
13. Life in the UK: Original 2
14. English Test/UKNARIC - For both myself and wife
15. Passport - Old and New
16. BRP - 3
18. Degree (if UKNARIC at point 14)
19. License copy and print from gov.uk website about no driving penalty points etc. (just to save time)
