Pls help,BRP replacement application question

Unfortunately my BRP card was stolen last weekend.
I'm applying for BRP replacement now.
There are aquestion on the form I'm not sure .Please help if you can.

"The name in which you made the application" -- Do I put my name there or the ?Because all my work visa was applied by my company before.

And do I need list all of my application in last 12 years? or just the latest?or only BRP related ?
