Wife takes husbands last name after marriage - How to apply for new BRP card

Hello,

Can anybody answer my question and provide link to reference documents ?

My wife want to take my Last Name (last name change after marriage). which application form she should use to apply for name change on Biometric Residence Permit (BRP) card ?

Immigration details:-

I came to UK in April 2011 on Tier1 General visa
I got Indefinite Leave to Remain (ILR) in UK in April 2016
I got married in India in August 2016
My wife got spouse visa (three years) in October 2016 and she arrived in UK in November 2016
I got British Citizenship in October 2017
