Hello, vinny,CR001, and the others
please any help in this

I have booked PEO appointment and printed out version 4/2018 which was as link when I received the payment confirmation , I have filled most of it, but I found that there are another version in the .GOV.Uk after 17th May, do I have to print the latest version again?, it is a 86 pages and I am applying for 2 people.
Also do I have to fill the PAYMENT DETIALS? I already paid as PEO, can I keep it blank?
Also do I have to include section 9 of this form as I do not need to fill this section?

You should not use the old version 21 days after the new version released. Or your application may be invalid.

You dont need to fill again.

If u are not applying through Tier1 you don't need to fill

Another question please, my job is in the shortage occupation list. I worked in my first employer as a full time job. After one year I signed a zero contract with another department in the same hospital do weekend work once a month (the same profession), this mean I had 2 pay slips from the same employer.
In addition my main department (the full time job) asked me to do work in another hospital to cover staff sickness and that was as emergency Saturday work, and was only for a few visit only. This hospital paid me independently and I paid tax return on the money I earned from that hospital. Do I need to provide any document regarding this work? this was part of work cooperation between the 2 hospitals.
After 2.5years I changed my employer (of course through the home office application). but the first employer wanted me to continue with them in zero contract until they find a staff to do the work. so I quit from the full time work and remained with them on zero contract, (the human resources confirmed to me that I am allowed to take 20 hours/ week in addition to my full time job. However I stopped going there since May last year but still with them on the Zero contract for any emergency work.
My questions : do I need to write all these details in a cover letter?, what documents I need to provide with this letter?. I have a letter from the first employer explaining my situation with the hospital, will this be enough?
