need information EU treaty (stamp4 eufam)expiring

HI I AM INDIAN national and my wife is eu citizen we got marry in 2013 and got stamp 4 eufam for 5 years and its expiring in 2019 so i am going to apply for EU3 form. me and my wife separted for abt 2 years (2015-2017 but still married) as she moved with her mom and i stay on same place. Then we move together again in 2017 on my same address where we used to live i am living on this address since last 6 years i have bills on me and her name(even for the period she moved out) more thn 5 years . she working all 5 years and still working with same company since 2015.
I have her all 5 years p21 statement from revenue office ..but so i have lease copies and bills on me and her name letters from tax office . but her bank statement is still on her mom address as we didn’t bother to change but now as i am seeing documents for EU3 they asking about bills on both name not the bank statement.
Queries

1 .so i in application should i mention her other address and time she she moved out ?
2. if immi asked her old bank statement should i submit bank her bank statement with other address ? now bank have our current address on them anyway
3. and she have 3 p60 but 5 p21 statements from each year that be fine ?
