The exemption is only for the following SOC doctors code 2211 and nurses code 2231. No, that is not permitted. You must be paid what is stated on the CoS and SOC.

Yes it is. Remember... it is the employer who puts out the advert for RLMT at the higher rate of 50k according to HO rules. I applied again, and they use this new RLMT to apply for COS. Nothing wrong in that I presume. I recall alot of us here have don't the same too.
