Tier 4 dependent visa - previous history

Hello Dear,

I am applying as Tier 4 dependent will my previous travel history effect my visa.
1. Have you made an application to the Home Office to remain in the UK in the last 10 years?
Yes, I have applied for Tier 1 Post study work in june 2011 and Tier 2 General on 15/07/2013, the issue is I don't remember the exact date of application when I have applied for Tier 1 post study work, will it be a problem if i mention nearby date.

2.Have you ever voluntarily elected to depart the UK?
Yes or No?
My tier 2 general leave to remain is curtailed, I have received a letter from home office
Your leave to reamin has been curtailed. You should leave the UK, your must leave the UK now, or you will be liable to be detained or removed. It also mentioned that I will not be removed for the first seven calender days after i receive this notice.Letter dated on 03/09/2015 and it came by post (if you received this notice by post you are counted as receiving it two working days after it was posted)
notice dated on 03/09/2015 (thursday), it says 2 working days friday and monday that will be 07/09/2015 and I left on 13/09/2015.
Does this notice come under -- Are you, or have you been subject to, an exclusion order from the UK?
