UK Exp & Past Earnings.. Pls help

Dear all,

Please help me with the following.

1) Are PBS-VAF9A & PBS-VAF9B the only application forms required for Tier-1 application? Please let me know if i have missed anything

2) I was in UK (on deputation from Indian Company) for the last 1 year. However am back in India before 2 months and applying for HSMP under Tier-1. But am not claiming any points for UK Work Experience or Past Earnings category?

However the application form collects details about UK National Insurance Number, UK Travel History, VISA details etc. Will that hinder any chances for my application getting approved.

3) While claiming points under past earning category, should we use the CTC amount or just the monthly gross salary for conversion. For eg, if my CTC as per increment letter is 5.7lpa. But my gross sal would be just 43k (given the PF etc amt not included in GROSS). Should i go for the complete 5.7lpa or just 5.16lpa (43*12) for conversion.
