IELTS answers in English Language section - Tier 2 (General) Application on Visa4UK

Hello Everyone,

I'm a little bit confused about how to answer the questions. I'm applying using Visa4UK. I read some previously posted answers but they contradict each other, so please help me if you are sure about the answer.

Awarding body: is it University of Cambridge or British Council? my test centre was British Council.
Title of qualification: International English Language Testing System
Level of qualification: CEFR Level C1 or Overall band 7.0?
Date of award: written beside the test report form number?
Award reference number: is it the UKVI number or the TRFN?

I just managed to make sure of the Awarding body through this document it is Cambridge English
Language Assessment.
and the Award reference number through this guid (page 23). it's the UKVI number.

Anyone knows what to write in the Level of qualification field? my scores are (8, 7.5, 6, 6)
