Renew of permanent residence card?

Hi ,
I need to renew my permanent visa but I was wondering if I need to put any details of the person that has sponsed me in the passed ?
I am a non eea

Do I need to fill section 1 C ?

You mean the PR card? Or the Residence Permit?
If it's PR the card is valid for 10 years - but PR is permanent so even without card you won't loose the status. Please clarify.
