documents required for dependant child and need some suggestion

Hello all,

I am on tier 1 ent ext visa,Can anyone help me by sharing the documents list for dependant child born in the UK.
And I have applied for his passport so can I book one day appointment now in advance as passport will take 2-3 weeks to come so if I get appointment after 1 month then I will not have to wait after receiving his passport.
But website ask me to put passport number for getting appointment and to pay NHS so can I enter passport application reference no. intead of passport no.


Please give me suggestion.
