Switching from ICT tier 2 PBS dependant to tier 2 general

Hi,

I am in the UK on a tier 2 ICT visa since Jan 2011. I was granted Tier 2 ICT entry clearance under rules in place before April 2011.

My wife joined me in Feb 2015 as my dependant with ICT tier 2 PBS dependant visa. She is currently working with an employer who may consider to sponsor her for tier 2 general visa provided it doesn't cause a break more than 4 weeks in the employment.

So my questions are

Q1) Can she switch to tier 2 general from tier 2 ICT PBS dependant visa from within the UK?
Q2) If answer to Q1 is NO then does she have to serve 12 month cooling off period?
q3) if answer to Q2 is NO then can she go back to her native country and apply for tier 2 general from there while she holds a valid dependant visa? and can that be done within 4 weeks through some fast track premium service?

3.Yes, but unsure about 4 weeks time frame. What salary is she being offered and what is her SOC? I leave other experts to answer this. You need to do more research and look for topics in the form where applicants discuss on salary threshold etc...

She is not in a job which comes under shortage occupation list but her company is ready to sponsor her and do an rmlt test to issue a cos. Is that possible?
