Spouse Visa Requirments

Hi All
This question is mainly to fellow members who applied and got their dependent visa from Pakistan,rather than some guru.
Please if anyone can share the full step by step procedure of applying wife's visa from Pakistan like
Maintenance is done, wife's passport and nikahnama done,
whats the fees and how do we pay
do we have to apply online from here and submit documents in Islamabad?
english language requirment? (my wife has a bachelor degree from pakistan)
whst exactly should i take as a main applicant with me to apply her visa
and if any additionalinfo will be much appreciated.
