EEA Residence card

Hi,

I am filling out application form for my mother who is applying for EEA residence card. She came to UK on 30/5/18 on family permit. My father is her sponsor. I am bit confused with couple of questions where they ask in section 5 if you are currently living together tell the date when you start living together (Should i put the date when they got married or should i put the date when she came to UK to live with my father). Second question is have you ever lived with your sponsor within or outside the UK. Please also tell how to answer this because she has lived with him outside UK but it is hard to put the dates my father use to visit her regularly in Pakistan. She has also lived with him couple of years in Italy. If we can't put the dates How should we answer this question?
