Gap between visas

Visa one: 28/08/2009 - 01/08/2010
Visa two: 04/09/2010-04/02/2012
Arrived on 22/09/2009, left on 30/06/2010, and then arrived on 25/09/2010.

Would this count as `continuous residence'?

In the text below, does it mean within 28 days for application only or including granted and re-entering the UK?
• leaves the UK with or without valid leave, but applies for new entry clearance within 28 days of their leave expiry date, is granted and re-enters the UK using that entry clearance.

Very appreciate your help.
