Period of evidence for cohabitation

Dear Experts

I am a British citizen since 2015 and was previously on Tier 1 visa. My wife, PBS dependant, is now applying for ILR this month using form Set (O) online.

1. In the online application, there is no mention of cohabitation proof. Do we still have to provide evidence? If so, is it for a period of 2 years or since the last grant of leave to remain (in her case, leave to remain was granted on Oct 2017 until 2020)? Where in the immigration rules can i find more information on this topic?

2. For a period of 6 months, we do not have any proofs for her, except a utility bill that says Mrs R (my surname), although she goes by a different name and surname. Will this be accepted?

3. As she is unemployed and dependent on me, do I need to furnish my P60, salary slips and bank statements?
