BRP-NTL Application Form

Hi all,

I have booked premium service - NTL application for this week. I have a confusion regarding the NTL application form. In the online service for premium appointment, a 'download link' saying NTL application is provided to print (after the payment). But when I printed, it has only my general details and not the same as original NTL application in the website. I understand I still need to complete the 21 page application and take that with me, right?
