waiting for dependent's Health surcharge refund and CT41G

Dear gurus and members,
I need your help in 2 cases
1) if is there any way to request for partially health surcharge refund, basically my T1 entrepreneur extension was approved in March 18, but my daughter's as a dependent's application was refused, we submitted fresh application and paid full health surcharge for her, but did not refunded partially yet. (she was granted visa in June 18).
2) I received my documents back in different times in different parcels, like at first approval letters for me and other dependents, than biometrics, after 2, 3 days other documents which were submitted at application submission time, that is why I spent much time to gather and give them in order then I realised that my company's CT41G was missing, what should I do now, can I request to get back somwhere?
