Questions regarding the Application for naturalisation as a British citizen form

Dear Sir / Madam,

I'm going to apply for British citizenship and have started filling in the Application for naturalisation as a British citizen form, but got stuck in certain sections that I'm uncertain about how to fill in. Unfortunately, the Guide AN and the Booklet AN don't answer to these questions.
I would appreciate if you could help or guide me to the right helpline regarding my questions.

Please see my questions below.

1.3 What is a given indefinite leave to enter / remain in the UK date? Does it apply to me if I'm an EEA nationa? What date to put in the boxes?

1.17 The address for the past 5 years of my stay in the UK includes the one current. What shall I put in the boxes for 'To:' if I'm still on the address?

1.24/1.28 How should I write my parents name in the boxes if their official name starts with their surname and continues with their first and middle name in their official passport and ID documents?

1.49 Currently I don't have a tax reference number as I"m employed by an EU public sector organisation and my salary is received from the EU. Shall I leave the boxes blank or use the last tax reference number I was given by my last UK employer in 2014?

1.50 On various occasions I was employed through recruitment agencies, so I believe I need to add them as employees. What address shall I write? Is it the address the recruitment agency is registered on or the branch address I was registered with?

2.2 When listing the dates of absences from the UK and counting the total number of days absent, do the dates for travel count or should be excluded?

Hi,

First question is do you have a Permanent Residence Card ((PRC)? If so when was it issued?

As an EU/EEA citizen you will be required to have a PRC, after exercising Treaty Rights for five years plus one year and meeting certain criteria, as a precursor to applying for UK citizenship, unless you have been here a very long time and may have applied for and received Indefinite Leave to Remain.

When did you first enter the UK?
