ILR after Descretionary Leave

Hello I like to enquire as to what form to fill for my settlement after 6 years discretionary leave.
My story is, I came to the uk 16years ago started as a visitor then became a student. 2006 was refused visa then in 2008 applied for asylum which was refused and all rights exhausted in 2009. I was told to report and kept on going to report at the reporting centre. In 2011 I made a fresh application for Indefinite leave to remain using Set O based on family life and long residency. 6 months later they granted me 3 years Descretionary Leave which was to be renewed once for another 3 years before they could consider giving me the indefinite leave to remain. Now it's due soon but I'm not sure which form to fill. Set (O) or FLR (DL) please help!
